New Patient Forms
New patients will be required to have their drivers’ license or official ID available. In addition, routine forms will need to be filled out. Please download the forms below, fill them out and bring them to your appointment. You may also create a login and fill these forms out online through our patient portal. This may minimize check-in times. Patients under 18 must be accompanied by a parent. Please DO NOT forget to bring your insurance cards at the time of your visit.
Please bring any recent X-rays, CAT scan, MRI scans (reports and actual images) to your appointment. This will minimize duplications of studies already performed.

Listed below are important things to be aware of that will help you prepare for your visit to our office.
- You must bring valid photo identification
- Please bring current Insurance Card or Insurance Information
- Referral, if required
- If your injury is work or auto-related, please notify our staff immediately and provide the following information.
- Contact name, phone number and claim number
- We must verify your claim is open and active before you can be seen by the doctor.
Patients under 18 cannot be seen without a parent or guardian present for every visit
If you have previous x-rays or diagnostic tests pertinent to the injury being evaluated, please bring those images AND reports with you.
Please be prepared to pay co-payments at each visit along with a pre-payment towards your deductible, if it has not been met. For your convenience we accept Cash, Check, Visa, MasterCard, American Express and Discover. We also offer Care Credit as an option for payment of medical expenses.
For patients whose deductible for the year has not been met, you will be expected to pay for services at the time of your visit. DEDUCTIBLES are costs that have been passed on to patients by their insurance carrier before any payment is made by your insurance company for your medical treatment. Only when the deductible for the year has been satisfied, does your insurance contract for provider reimbursement kick in.
